Community posting testing area to show the workflow associated with how a user from the public would submit their information, where it would be stored, and how it could be added to other parts of the website.

A pre-defined form called "Health Declaration" has been added to show a fully fleshed out form that would allow for certain types of data input (e.g. dates, names, descriptions, etc).
